Need an Alabama birth, death, marriage or divorce certificate?

You may now apply for an Alabama Vital Record at 6 Health Department sites. It's easy. Fill out an application at the site, give payment and application to the clerk.

At the Main Health Department, you will only have a 15 to 20 minute wait (...unless a manual search is required which takes more time). If you visit a satellite site, you will be mailed the certificate within 2 business days.

Fees for certificates are $15.00 for the first copy and $6.00 for each additional copy when ordered at the same time.

Personal checks or money orders for the exact amount payable to the Mobile County Health Department are accepted. Drivers License and expiration date must be on all checks.

For name change or any other vital record issues, please visit the main Health Department at 251 North Bayou St. For further information, call the Vital Records Office at (251) 690-8150, (251) 690-8113 or write Center for Health Statistics, P.O. Box 5625, Montgomery, AL 36103-5625 or call (334) 206-5418.
